    • Print and fill-out the form:
    • Submit the filled-out form to your Supervisor/Director for signature of no objection for issuing your passport.
    • Submit the signed form to the Faculty and Personnel Affairs for approval.
    • Once approved, submit the completed form to the person in charge of passport for withdrawal.
